Revised Survivorship Agent’s Report – Effective 08/16/21

Effective Monday, August 16, 2021, the Survivorship Agent’s Report (ORD 114120) will be revised to support eDelivery. The form will carry a 2021 revision date. The Survivorship Agent’s Report (ORD 114120 SURV) will be revised as follows:

• Revise Section B. Producer Information to include Producer e-mail for electronic policy delivery.
• The form will carry a 2021 revision date.

The revised form will be available on the PruXpress website effective Monday, August 16, 2021.
