Revised Consent for Electronic Policy Delivery Effective 05/16/22

Effective Monday, May 16, 2022, the Consent for Electronic Policy Delivery (ORD 115309 2022) will be updated to add information for a second proposed insured to support use of survivorship applications with eDelivery.  This change had been previously communicated for implementation on April 11, 2022.

Revised Consent for Electronic Policy Delivery

The Consent for Electronic Policy Delivery (ORD 115309 2022) will be updated as follows:

• Add Second Proposed Insured name line.
• Add Second Insured details for “Electronic Policy Delivery Contact Information”.
• Add Second Proposed Insured signature and date.
• The form will carry a revision date of 2022.

The revised form is required with applications with a Part 1 date of Monday, May 16, 2022, and later. The current form can continue to be accepted for use with non-survivorship applications.

This change was previously communicated for implementation on April 11, 2022.

The revised form will be available on the PruXpress website and iGo effective Monday, May 16, 2022.
