New Utah Form – Effective 02/22/2021

Effective Monday, February 22, 2021, Utah recently adopted a new regulation that allows policyholders to identify a secondary addressee to receive a notice of cancellation for nonpayment of premium; as a result, a new Utah state-specific Secondary Addressee Notice (ORD 112674 UT 2020) has been created and will be available for use.

The Utah Secondary Addressee Notice is required to be provided to the applicant at point of sale. The form will be available on the PruXpress website effective Monday, February 22, 2021.
