Death Claim Form Revision

An updated Individual Life Insurance Claim form (COMB 38B Ed. 01/2023) was recently released and will be sent with all new first notifications of death. This version includes new regulatory requirements for tax withholding, Department of Labor language, and fraud warnings. Additional changes were also made to add the check box back on to request “Field Office” delivery of the claim check and to include a default payment option notification.

One of the more significant changes was to Section 1. “About You.” If the beneficiary is completing the form, they no longer have to check a box stating their relationship. The check boxes are only required if the person completing the form is someone other than the beneficiary.
