Changes in Credit Card Payments

Effective June 28, 2023, due to our vendor’s processing change, we will no longer be able to accept credit cards as a method of payment. This applies to all payments, including first premium payments.   
  
We can accept personal checks, cashier’s check, and electronic check or eCheck. Checks can be made payable to OneAmerica or to The State Life Insurance Company^®.  
  
The Credit Card Authorization form, I-34819, will be discontinued and we will no longer accept it after June 27. Please destroy any printed stock, it will not be replaced.
