Enhanced Electronic Funds Transfer (EFT) Form

We are excited to announce we have updated the EFT form (L-1683 rev. 12-20) to include an additional payment option for one-time premium payments. In addition to the new payment option, other enhancements were made to align the formatting of the form across all our business units and in an effort to continue to decrease the rate of incorrect forms received.
