Reminder: Living Promise Application Form

In the fall of 2023, we made some exciting changes to United of Omaha’s Living Promise whole life product and as we start the new year, please be sure you are using the updated application form. We continue to receive old applications forms for Living Promise and we want to be sure you are using the correct one.

The new paper application form number is ICC23L681A and is currently available in Forms and Materials, or through your normal ordering channels. The new application form is now required to be used for all applications. If an old form is received, you will be asked to resubmit using the new application.
