New Form Requirement in PA, TX, UT

Effective Wednesday, May 22, 2024, the Long-Term Care Insurance Personal Worksheet will be required for submissions in Pennsylvania, Texas, and Utah that include John Hancock’s Long-Term Care rider. This form is a state requirement and helps ensure the rider is the right solution for the proposed insured.

• The Long-Term Care Insurance Personal Worksheet will be available on JHSalesHub.com through the forms vendors we support and in digital eApplication platforms beginning May 22
• The form must be signed and dated on or before the application signed date
• Starting May 22, we cannot approve or issue policies in PA, TX, or UT that include the LTC rider until we receive this new requirement
