Applicant must provide one of the following:

• A copy of their Permanent Residence Card (Green Card)
• A copy of their Visa (temporary Visa NOT accepted)
• Or ITIN number with a

   2. Driver’s license
   3. Consular card OR
   4. Passport book or card
