The following updated life insurance forms are available at www.allianzlife.com:

• Temporary Insurance Agreement form
• HIPAA Authorization form

Check the bottom right corner of the form you’re using; if the date is earlier than 11/2023, it is an old version and will not be accepted after December 31, 2023. As of January 1, 2024, 11/2023 versions will be required.
