Revised MN Important Notice Regarding Replacement – Effective Monday, 12/11/2023

Effective Monday, December 11, 2023, the suicide clause will be updated on the Minnesota state specific Important Notice Regarding Replacement form (ORD 89216 MN) in compliance with Minnesota Senate Bill 2744.

The revised form is required with a Part 1 date of 12/11/2023 and later. Older versions of the form will continue to be accepted until Monday, February 12, 2024. The updated form will be available on PruXpress on Monday, December 11, 2023.
