Revised PruLife Founders Plus UL Supplement Effective 04/19/21
Effective Monday, April 19, 2021, the PruLife Founders Plus Supplement (ORD 96200-2020 PFP) will be revised to include the new Goldman Sachs Voyager Indexed Account. A new PruLife Founders Plus Disclosure (ORD 115809-2020 PFP) will be introduced for use with the revised product. Please refer to the Ready to Sell List for state approval information.
Revised PruLife Founders Plus Supplement
The PruLife Founders Plus Supplement (ORD 96200-2020 PFP) has been revised as follows and will be used with the April 2021 PruLife Founders Plus product with issue basis 307 and 306.
- Rename the “Plus 100 Account” to “S&P 500® Indexed Account”.
- Add the “Goldman Sachs Voyager Indexed Account”.
- Add transfer options in the “Designated Transfers (Optional)” section.
- Introduce state-specific forms for the following states:
o Alabama (ORD 96200-2020 PFP AL)
o Arkansas (ORD 96200-2020 PFP AR)
o Colorado (ORD 96200-2020 PFP CO)
o District of Columbia (ORD 96200-2020 PFP DC)
o Florida (ORD 96200-2020 PFP FL)
o Hawaii (ORD 96200-2020 PFP HI)
o Louisiana (ORD 96200-2020 PFP LA)
o Maryland (ORD 96200-2020 PFP MD)
o Maine (ORD 96200-2020 PFP ME)
o New Jersey (ORD 96200-2020 PFP NJ)
o New Mexico (ORD 96200-2020 PFP NM)
o New York (ORD 96200-2020 PFP NY)
o Ohio (ORD 96200-2020 PFP OH)

The forms will carry a revision date of 2020.
When submitting the long application, the Supplement must be completed when applying for the April 2021 Pru Founders Plus product with issue basis 307 and 306.
When submitting the Worksheet, please complete section O, subsection 1B and the Supplement will be included in the policy at issue.
New Pru Founders Plus Disclosure
The new Pru Founders Plus Disclosure will carry form number ORD 115809-2020 PFP. This form must be completed when submitting the long application or the worksheet.

The new form will be available on the PruXpress website Monday, April 19, 2021.
